Keep in mind that starting with 2011 taxes Brokers are required to supply you with your cost basis and gains and losses for equities, including wash sales (unless you elected mark to market as a trader). For instruments other than equities I believe it is 2012.
Just search the internet for Schedule D Stock Gains and Losses.
There are at least 5 to 6 decent ones.
